This new eFI tool can be found under ‘Open Evolution Reports’.

This useful tool has been developed by Leeds University to help practices identify patients who are living with moderate to severe frailty.

This new eFI tool can be found under ‘Open Evolution Reports’.

The tool will indicate those patients who may have:

  • Moderate frailty
  • Severe frailty
  • Severe frailty with an annual medication review
  • Severe frailty and are recorded as having had a fall in the preceding twelve months
  • Severe frailty and have provided explicit consent to activate their enriched Summary Care Record

In the next phase, a patient assessment tool will be delivered which will write back to the Open Evolution records and which will boost the care you provide to these groups of patients.
